Executive Summary:
This item provides a staff update on the Novel Coronavirus threat and current response activities. In addition, this item also includes a Pandemic Annex to the Sonoma County Operational Area Emergency Operations Plan for your Board’s review and approval. In order to maximize the effectiveness of the County’s Emergency Operations Plan (EOP), the Department of Emergency Management (DEM) has developed an annex providing detailed guidance for response to a Pandemic event.
Discussion:
Coronavirus is an outbreak of respiratory disease caused by a novel (new) coronavirus that was first detected in China and which has now been detected in almost 70 locations internationally, including in the United States. The virus has been named “SARS-CoV-2” and the disease it causes has been named “coronavirus disease 2019” (abbreviated “COVID-19”).
On January 30, 2020, the World Health Organization declared the outbreak a “public health emergency of international concern”. On January 31, 2020, the Secretary of the U.S. Health and Human declared a public health emergency (PHE) for the United States. The Governor of California proclaimed an emergency on March 5, 2020. The County of Sonoma Director of Emergency Services proclaimed a local emergency and the Public Health Officer declared a Public Health emergency on March 2, 2020.
Source and Spread of the Virus
Coronaviruses are a large family of viruses that are common in people and many different species of animals, including camels, cattle, cats, and bats. Rarely, animal coronaviruses can infect people and then spread between people such as with MERS-CoV, SARS-CoV, and now with this new virus (named SARS-CoV-2).
The SARS-CoV-2 virus is a betacoronavirus, like MERS-CoV and SARS-CoV. All three of these viruses have their origins in bats. The sequences from U.S. patients are similar to the one that China initially posted, suggesting a likely single, recent emergence of this virus from an animal reservoir.
Currently, the CDC Pandemic Intervals Framework places the Coronavirus in its 3rd phase or “Initiation” Phase - just before the 4th or definitive phase of widespread infection - the “Acceleration Phase”. 1
As of March 4th, there over 92,000 confirmed cases in 80 countries with 3,168 associated deaths.
Pandemic Annex
In order to maximize the effectiveness of the County’s Emergency Operations Plan (EOP), the Department of Emergency Management (DEM) had developed an annex which provides detailed guidance for response to a Pandemic event.
The annex establishes a common multi-stakeholder preparedness and response structure to an imminent threat of a disease outbreak or pandemic affecting the Sonoma County Operational Area (OA). It is a supporting document to the wider OA Emergency Operations Plan (EOP) and aligns with the State of California Standardized Emergency Management System (SEMS) as well as the National Incident Management System (NIMS). This annex addresses the following objectives:
• Serve as an augmented operational structure to the Sonoma County Department of Health’s (DHS) infectious disease plan
• Explain the pandemic threat and its potential to have a major impact in the community
• Define the various risk levels of a disease outbreak/pandemic and recommended actions
• Identify the OA partners involved and the critical operational response areas of coordination
• Limit the impact of a disease outbreak/pandemic in the community by:
o reducing the potential morbidity and mortality rates
o minimizing disruption of social and medical services
o ensuring timely and appropriate information is disseminated to the public
This item requests that the Board receive a staff update on the Novel Coronavirus threat and current response activities and approve the Pandemic Annex to the Sonoma County Operational Area Emergency Operations Plan.
