Executive Summary:
Mandated by law, the need for a Medical Director is required for both Juvenile Hall, which is operated by the Probation Department and Valley of the Moon Children’s Center (VOMCC), which is operated by the Human Services Department (HSD) to provide medical oversight of staff working within these clinical programs. Additionally, the Medical Director writes, edits, and updates the standardized procedures for the nursing staff working in these facilities. The Medical Director works closely with Department of Health (DHS) program manager and Deputy Health Officer to ensure that state and federal mandates pertaining to the health care provided at both facilities are met.
In April 2022, DHS (or “the Department”) released a request for proposals (RFP) to clinical healthcare organizations that maintain a focus on pediatric and adolescent care to provide Medical Director services to Juvenile Hall and VOMCC. After review of the two submitted proposals, the RFP evaluation team, comprised of Probation and DHS staff, recommend entering into an agreement with Santa Rosa Community Health Centers (SRCHC) for the needed services. The proposed agreement is for a three-year term beginning July 1, 2022.
DHS has an executed memorandum of understanding with HSD and the Probation Department to provide medical oversight to the two programs. The medical services are the responsibility of DHS who conducted the RFP process.
Discussion:
Under the proposed agreement, SRCHC will provide guidance and consultation to DHS Registered Nurse staff, provide direct patient care to patients with complicated case-files, review medical protocols annually, revise protocols as needed, provide training to staff approximately four times per year, be available for occasional calls from program staff after regular business hours, assist current program administration with systems improvement to enhance current medical services, and participate in monthly staff meetings.
In April 2019, the Department released a request for qualifications (RFQ) for Medical Director services at Juvenile Hall and VOMCC. No responses to the RFQ were received. The Department contacted SRCHC to discuss programmatic needs at the facilities and determined that SRCHC could provide the needed services. Based on discussions with County Purchasing, the Department requested, and the Purchasing Agent approved, a single-source waiver for an agreement with SRCHC. An executed agreement with SRCHC for Medical Director services at both Juvenile Hall and VOMCC has been in place since July 2019 and will expire at the end of June 2022.

Funding for the agreement with SRCHC will be fully covered through a memorandum of understanding (MOU) between DHS and HSD and the Probation Department. DHS maintains the agreement with SRCHC and receives reimbursement from VOMCC and Probation for services rendered to each of the programs.

Narrative Explanation of Fiscal Impacts:
Funding for this agreement will come from an MOU worth $307,092 over three fiscal years with the Probation Department. $98,376 and $102,312 will be spent in FY 22-23 and FY 23-24, respectively. For FY 24-25, the cost for services will be $106,404.
The HSD funding is a subset of the Family, Youth & Children’s Services program and is already budgeted for 22-23. Funding is a mix of 1991 Realignment, 2011 Realignment and General Fund. The Probation funding is General Fund.
