A) Authorize the Director of Health Services, or designee, to execute a revenue agreement with First 5 Sonoma County to accept revenue to provide funding to support the Department of Health Services Nurse-Family Partnership program in the amount of $300,000 for the period July 1, 2021 through June 30, 2022.
B) Authorize the Director of Health Services, or designee, to execute a revenue agreement with First 5 Sonoma County to accept revenue to provide funding to coordinate and support the Department of Health Services countywide Home Visiting Collaborative in the amount of $13,326 for the period July 1, 2021 through June 30, 2022.

Executive Summary:
Nurse Family Partnership (NFP) is an intensive evidence-based community health program designed to help transform the lives of vulnerable mothers, pregnant with their first child, through support from a home visiting nurse. It is a nationally known program that is listed on the Upstream Investments’ Portfolio as a Tier 1 evidenced-based program. First 5 Sonoma County has been providing funding for the Department’s Nurse-Family Partnership program since implemented in 2010. This item requests approval of an agreement with First 5 Sonoma County for continued funding to support the Department’s Nurse-Family Partnership program in the amount of $300,000 for fiscal year 2021-2022.
The Home Visiting Collaborative is countywide network of home visiting program directors for the purpose of quality improvement and coordination of services. This item requests approval of an agreement with First 5 Sonoma County for funding to support the Department’s Home Visiting Collaborative in the amount of $13,326 through June 30, 2022.
Discussion:
Nurse-Family Partnership is an intensive evidence-based community health program designed to help transform the lives of vulnerable mothers, pregnant with their first child, through support from a home visiting nurse. It is a nationally known program that is listed on the Upstream Investments’ Portfolio as a Tier 1 evidenced-based program. The Nurse-Family Partnership program was implemented locally in 2010. The Department convenes a countywide Home Visiting Collaborative, which includes the Nurse-Family Partnership program and other Department home visiting programs, as well as other government and community-based agencies providing home visiting services. The goal of the Home Visiting Collaborative is to provide coordination of services and improve the quality and access of Sonoma County home visiting programs.
Through fiscal year 2018-2019, First 5 was administratively located within the Department of Health Services and provided funding for the Nurse-Family Partnership program via a memorandum of understanding not requiring Board approval. On July 1, 2019, First 5 Sonoma County became a legal public entity separate from the County of Sonoma. As such, agreements between First 5 Sonoma County and the County of Sonoma require Board approval.
This item requests approval of an agreement with First 5 Sonoma County for the County to receive revenue in the amount of $13,326 through June 30, 2022. The funding will be used to support .05 FTE of the Healthy Communities Section Manager who will support planning and coordination efforts of the home visiting programs in Sonoma County.
This item also requests approval of an agreement with First 5 Sonoma County for the Department of Health Services to receive revenue in the amount of $300,000 through June 30, 2022. The funding will be used to support 0.75 FTE of a Public Health Nurse Supervisor who will provide clinical supervision to a team of 3.80 FTEs of Public Health Nurse home visitors. The Public Health Nurse team will each carry a caseload of up to 25 families, providing home visiting services in compliance with the NFP service model. The funding will also support 0.95 FTE of a Senior Office Assistant to provide data entry and reporting through an on-line database. This funding will support additional expenses related to NFP required training and technical assistance, operating costs, and client related expenses.
Prior Board Actions:
On May 19, 2020, the Board approved a revenue agreement with First 5 Sonoma County to accept revenue for the Department of Health Services Nurse-Family Partnership program in the amount of $1,200,000 for the period of July 1, 2019 through June 30, 2021.
