A)                     Adopt a resolution authorizing budgetary adjustments to the Fiscal Year (FY) 2021-22 budget in the amount of $332,572 to reflect increased funding from the State of California Department of Aging. (4/5 Vote Requirement)

B)                     Authorize the Director of the Human Services Department to execute amendments to State FY 2021-2022 funding for the following three contracts:

i.                     Increase the current contract with Council on Aging by $143,366 for a new total not to exceed $1,550,838;

ii.                     Increase the current contract with Petaluma People Services by $38,272 for a new total not to exceed $412,312; and

iii.                     Increase the current contract with Senior Advocacy Services by $71,495 for a new total not to exceed $874,297.

C)                     Adopt a resolution authorizing the Director of the Human Services Department to make budgetary adjustments to the FY 2021-22 budget in the amount of $95,327 to fund Medicare support services for older adults with increased funding from the State of California Department of Aging, to execute the Medicare Improvement for Patients and Providers Act (MIPPA) Standard Agreement #MI-2122-27 with the State of California Department of Aging, and to execute future amendments to adjust for revenue changes.

Executive Summary:

The Human Services Department (HSD) requests a FY 2021-22 budget adjustment in the amount $332,572 and authorization for the Director of HSD to execute amendments to existing contracts with Council on Aging, Petaluma People Services Center, and Senior Advocacy Services. This reflects a federal funding increase of $332,572 received from the State of California Department of Aging (CDA) as part of the $2,773,610 approved by the Board of Supervisors on June 8, 2021 for the Area Agency on Aging to fund services for Fiscal Year (FY) 2021-22. As part of HSD’s FY 2021-2022 adopted budget, the Board approved an on-going  General Fund allocation of $250,000 to support older adult nutrition programs that provide both home-delivered meals and lunches at congregate settings as well as nutrition education. HSD has also allocated $200,000 of its Realignment funding allocation to support older adult nutrition programs.  

 

The HSD, Adult and Aging Division is requesting the Board of Supervisors to adopt a resolution authorizing the Director of HSD to execute a CDA revenue contract to accept federal funding designated to support the Medicare Improvement for Patients and Providers Act (MIPPA). MIPPA expands awareness of Medicare benefits for low income older adults and financial assistance for Medicare premiums.

 

Discussion:

The Sonoma County Area Agency on Aging (AAA) is responsible for planning, advocating, and providing funding for services that support the needs of older adults, adults with disabilities, and their caregivers throughout Sonoma County. The CDA distributes federal funds from the Older Americans Act (OAA) to local AAAs who then contract with local service providers that assist older adults and people with disabilities to remain in the home of their choice for as long as possible.

 

The initial funding for AAA service provider contracts that began on July 1, 2021 for state FY 2021-2022 was based on the funding amount received in FY 2020-2021. This is because the OAA funding allocation from CDA for FY 2021-2022 was not received early enough to allocate the actual funding amounts to be completed by July 1, 2021. In order to reconcile the service provider contracts to match the actual OAA allocation received for FY 2021-2022, it is necessary to complete the amendments discussed in this board item.

 

Funding amendments with Council on Aging and Petaluma People Services Center will utilize both OAA and County funding to support older adult nutrition programs.

 

The funding amendment for Senior Advocacy Services will utilize additions to the initial OAA allocation and the funding from MIPPA standard agreement.

 

1.                     Council on Aging contract: increase the current contract by $143,366 for a total not to exceed $1,550,838

2.                     Petaluma People Services Center contract: increase the current contract by $38,272 for a total not to exceed $412,312

3.                     Senior Advocacy Services contract: increase the current contract by $71,495 for a total not to exceed $874,297

 

See Attachment 7 for the breakdown of the total increases per service provider and program.  Below are descriptions of the programs where funding amounts have changed.

 

Council on Aging and Petaluma People Services Center

Adult Day Care Program

The Adult Day Care program provides day care services for older adults who are functionally or cognitively impaired by providing the emotional, mental and social stimulation that is necessary to help the participants remain independent in their efforts to age in place and stay healthy. The program provides professional, stimulating, and accessible programming in a safe and comfortable home-like setting appropriate to varying levels of individualized needs. A nutritious lunch and a light snack are also offered.

 

Nutrition Programs - Home Delivered Meals and Senior Lunches at Congregate Sites

Home delivered meals are designed to increase the nutritional intake of homebound or isolated adults 60 and older who may have become homebound due to increasing age or short-term/long-term health problems. The program provides nutritional support through the delivery of one meal per day to assist in maintaining health, independence and ability to remain at home.

 

The senior lunch program (congregate meals) provides a nutritionally balanced meal and social contact that provides positive motivation for self-care for adults 60 and older who may often eat poorly on their own and may become lonely and depressed in isolation.

 

Senior Advocacy Services

Medicare Improvement for Patients and Providers Act (MIPPA)

MIPPA provides funding to help Medicare beneficiaries apply for two valuable benefits: the Medicare Part D Low-Income Subsidy "Extra Help"; and the Medicare Savings Program, providing financial assistance in paying Medicare premiums.

 

MIPPA funding is used to provide expanded outreach, awareness, and education efforts to older individuals currently receiving Medicare who are unaware of these valuable benefits.  Services include targeted education to assist Medicare beneficiaries to apply for benefits if they are not currently receiving them.

 

MIPPA funding is allocated on a year-by-year basis, is not guaranteed each year, and is based on a modified federal fiscal year (FFY) funding cycle of September 1 through August 31. The County of Sonoma contracts operate on the state fiscal year cycle of July 1 through June 30. To account for the discrepancy in timelines, 10 months of the MIPPA funding (September 1 through June 30) is allocated to the contract in this board item with the remaining 2 months (July 1 through August 31) to be allocated in next FY 2022-2023.

 

The AAA has received MIPPA funding for the previous five FFY cycles in the following amounts:

FFY 2018-20: $120,332

FFY 2017-18: $63,960

FFY 2015-17: $37,763

Narrative Explanation of Fiscal Impacts:

The Human Services Department requests an adjustment to the FY 2021-22 budget to increase appropriations in the total amount of $332,572: $143,366 for Council on Aging; $38,272 for Petaluma People Services; $71,495 for Senior Advocacy Services; and $79,439 for MIPPA.

 

The current contracted amount budgeted in FY 2021-22 prior to the requested amendments is as follows: Council on Aging $1,103,245; Petaluma People Services $310,968;  Senior Advocacy Services $802,802; and MIPPA $60,166 for a total FY 2021-22 budgeted expenses of $2,277,181.

 

FY 2022-23 will increase by $15,888 for MIPPA. MIPPA funding for the period of September 1, 2021 through August 31, 2022 will be split in two portions: $79,439 in FY 2021-22 (10 months) and $15,888 in FY 2022-23 (2 months) to accommodate the federal fiscal year.

 

FY 2021-22 increased appropriations totaling $332,572 will be funded by $79,439 in MIPPA funding and a federal funding increase of $253,133 received by the California Department of Aging as part of the $2,773,610 approved by the Board of Supervisors on June 8, 2021, Item 31, Resolution# 21-0250 for the Area Agency on Aging to fund services for Fiscal Year 2021-22.
