Adopt a resolution authorizing the Director of the Human Services Department to execute the Supplemental Nutrition Assistance Program Education Agreement SP-2122-27 with the California Department of Aging, to accept $94,018 in revenue to fund evidence-based nutrition education and obesity prevention for older adults, and to execute future amendments to adjust for revenue changes.

Executive Summary:

The Human Services Department (HSD) Adult and Aging Division is requesting that the Board of Supervisors adopt a resolution and authorize the Director of HSD to sign a contract to accept funding from the California Department of Aging (CDA) to fund the Supplemental Nutrition Assistance Program Education for older adults beginning on October 1, 2021. Funding will enhance healthy lifestyles through increased physical activity and nutritional education.

 

Discussion:

The Supplemental Nutrition Assistance Program Education (SNAP-Ed), now referred to as CalFresh Healthy Living in California, is the nutrition education and obesity prevention component of SNAP (formerly known as Food Stamps). The overarching goal of SNAP-Ed is to promote healthy living through increased physical activity and nutritional education. The CDA is the recipient of the SNAP-Ed funding from the United States Department of Agriculture, and is a pass-through for contracting with statewide Area Agencies on Aging for implementing local evidence-based interventions.

 

SNAP-Ed funding is based on the federal fiscal year (FFY) funding cycle and allocated based on the state fiscal year cycle, resulting in $94,018 in funding for FFY 2021-22 (October 1, 2021 through September 30, 2022). The SNAP-Ed funding will be used to provide A Matter of Balance program and to deliver Food Smarts for Adults.

 

A Matter of Balance is a nationally recognized evidence-based program designed to help older adults age 60 and older avoid falls and increase activity levels. The program is delivered by trained coaches who received ongoing mentorship and periodic continuing education from the county’s team of Master Trainers. Sessions support older adults in learning how to identify fall risks by addressing issues regarding medications; providing problem-solving strategies to address behaviors and habits that contribute to falls; incorporating simple exercises to promote strength, flexibility and balance; and advising older adults on how to access community resources for added support.

 

Food Smarts for Adults is an evidence-based direct education intervention designed to support healthy behavior change in the areas of healthy eating, food safety, cooking, and food resource management. The learner-centered, multi-session nutrition and cooking program has several flexible lesson plans to fit the needs of older adults participating in a variety of settings.

September 17, 2019: Board approved Resolution #19-0369 authorizing the Director of HSD to execute an agreement for SNAP-Ed funding.

October 16, 2018: Board approved Resolution #18-0434 authorizing the Director of HSD to execute an agreement for SNAP-Ed.

October 16, 2018: Board approved Resolution #18-0433 authorizing the Director of HSD to execute an agreement for Medicare Improvement for Patients and Providers Act (MIPPA) funding.

The total amount budgeted in FY 2021-2022 for SNAP-Ed is $70,514

 

The California Department of Aging issued Standard Agreement SP-2122-27 awarding SNAP-Ed funding of $94,018 for the period 10/1/2021 through 09/30/2022 to be divided into two allotments: $70,514 to fund FY 2021-2022 and $23,504 to fund FY 2022-2023.

 

Program expenses are fully covered by funds from the California Department of Aging. There is no match required from County General Fund.
