Services include assistance with planning ahead for long-term care needs for the purpose of preserving continuity of health coverage. The target populations served by HICAP includes older adults, other Medicare beneficiaries including individuals with disabilities, and those imminently eligible who are seeking information related to medical insurance coverage options.

Sonoma County’s AAA serves as the lead agency under a memorandum of understanding  and provides HICAP functions for the AAA planning and service areas of Marin, Lake, Mendocino, Napa and Solano counties.
