A) Accept revenue in the amount of $143,750 from California Department of Aging for the Dignity at Home program. Funds will be accepted in FY 2019-20 for use in FY 2020-21.
B) Adopt a resolution authorizing the Director of Human Services to execute a contract with the State of California Department of Aging to receive funding for the Sonoma County Area Agency on Aging for a total amount of $143,750 for the term 7/1/20 through 6/30/21 and to execute future amendments to adjust for revenue changes.

Executive Summary:
The Human Services Department received funding for the Dignity at Home Fall Prevention Program (DAHFPP) from the California Department of Aging requiring a resolution be adopted by the Board of Supervisors to receive funding.
Discussion:
The Dignity at Home Fall Prevention Program was developed as part of the Governor’s Budget Act 2019, Assembly Bill (AB) 74, Chapter 23, Appropriation 4170-101-0001, Schedule 2, Provision 4. This is a program providing grants to Area Agencies on Aging (AAA) to provide fall and injury prevention information, education, referral services, equipment, assessments, services, materials to the eligible population. The eligible population means individuals who are sixty (60) years of age or have disabilities or whose adjusted household income does not exceed eighty (80) percent of the area median income and who are at risk of falling or institutionalization.
Among older adults, falls are the leading cause of injury-related deaths and are the most common cause of non-fatal injuries and hospital admissions for trauma. Falls in older adults are the leading cause for head injuries and broken hips. An older adult is seen in an emergency department for a fall-related injury every 11 seconds and every 19 minutes an older adult will die of a fall related injury. The financial toll for older adult falls is expected to increase every year. Forty percent of nursing home admissions are due to injuries related to falls with 40% of those admitted never returning to independent living. Even falls without injury can cause fear of falling, leading to physical decline, depression and social isolation. Research suggests that older adults underreport falls due to embarrassment, fear, or avoidance.
By accepting revenue supporting the Dignity at Home Falls Prevention Program, the County of Sonoma will strengthen systems and serve as a leader in the statewide effort to increase education and awareness about the importance of fall prevention for older adults.

Narrative Explanation of Fiscal Impacts:
There is no General Fund impact. The total appropriation in the amount of $143,750 for FY20-21 is from the California Department of Aging (CDA). The funds have been made available by CDA starting 02/01/2020 with authorization that they can be expended in FY 2020-2021 which HSD will now do. The expense appropriation will be posted to subsection 24030157 (Adults Grants) and account code 53012 (Support Care of Persons).
