Executive Summary:
The Human Services Department receives the State Department of Aging funding on an annual basis, and the State requires that each grant have a separate resolution adopted by the Board of Supervisors to receive the funding.
The funding from First 5 Sonoma County is for the Road to Early Achievement and Development of Youth project. Built on the premise that prevention focused policies and interventions have long-term impacts, the Road to Early Achievement and Development of Youth (the READY Project) is a unique cross-sector partnership focused on increasing access to quality early learning experiences and facilitating the transition of young learners from early childhood to kindergarten. The READY Project is implemented by the Human Services Department, 10 local school districts, and numerous early learning partners.

California Department of Aging Revenue
The Human Services Department (HSD) requests that the Board adopt two resolutions authorizing the Director of Human Services to sign contracts with the State of California Department of Aging to receive funding for the Area Agency on Aging Area Plan ($2,350,589), and the Multipurpose Senior Services Program ($685,600) for a total amount of $3,036,189 for the 2019-20 fiscal year. Both resolutions request delegated authority for the Director Human Services to execute future amendments to adjust for revenue changes. The Department receives this funding annually.
The Area Agency on Aging receives Older Americans Act funding from the California Department of Aging through separate Standard Agreements. The State of California Department of Aging requires the Board to formally accept the funding through the adoption of a resolution for each contract as follows:
- The Human Services Department is designated as the Area Agency on Aging. The Sonoma County Area Agency on Aging Area Plan Standard Agreement #AP-1920-27 with the California Department of Aging for FY 2019-20 is in the amount of $2,350,589. Funds will be used for Area Agency on Aging operations, direct and contracted services with community agencies to provide senior nutrition, supportive services, health promotion, family caregiver services and elder abuse prevention. This funding also supports existing staff and operational costs.
- HSD receives funds from the California Department of Aging to operate the Multipurpose Senior Services Program with the goals of preventing placement in nursing facilities and fostering independence and dignity for older adults. The program provides case management services to Medi-Cal eligible older adults who are frail and at risk of institutionalization. The Standard Agreement #MS-1920-11 with the California Department of Aging for FY 2019-20 is $685,600. Funding supports existing staff and administration for the program.
First 5 Revenue
The Road to Early Achievement and Development of Youth Project (READY Project) has been funded by First 5 Sonoma County since 2013 and seeks to improve the quality of early childhood education to help bridge the gap between early childhood education and K-12 schools. The new funding is for $300,000 for two years beginning July 1, 2019. The Sonoma County Human Services Department Planning, Research, Evaluation and Engagement Division provides overall project management. The funding will pay for a portion of a Program Planning & Evaluation Analyst and includes funds to work with school staff who participate in Project activities.
The READY Project provides training and technical assistance to early child care providers in Sonoma County in support of kindergarten readiness by collecting information on child readiness through the use of the Kindergarten Student Entrance Profile evidence-based tool. The continued funding will support expanded use of data collected over the last few years and expand support to teachers at schools throughout Sonoma County to integrate school readiness ratings to inform practice and instruction in the classroom.

the Department of Health Services for the Human Services Department to receive $1.5
million to implement the Road to Early Achievement and Development of Youth project.

Narrative Explanation of Fiscal Impacts:
Revenue from the California Department of Aging contracts and $150,000 from First 5 Sonoma County is included in the Human Services Department’s FY 2019-20 adopted budget. The remaining $150,000 will be added to the Department’s FY2020-21 budget. There is no General Fund match.
