A)                     Authorize the Director of Health Services, or designee, to execute an agreement with Santa Rosa Community Health Centers for a term of February 1, 2025, through June 30, 2026, to perform Sexually Transmitted Diseases Collaborative Services for a total not-to-exceed amount of $249,455.

B)                     Authorize the Director of Health Services, or designee, to execute an agreement with Face To Face/Sonoma County AIDS Network for a term of February 1, 2025, through June 30, 2028, to perform Hepatitis C Virus Prevention and Control Services for a total not-to-exceed amount of $366,972.

C)                     Authorize the Director of Health Services, or designee, to execute modifications to the agreements in this Board Item which do not significantly change the scopes of service and/or to increase the total not-to-exceed amount in order to address increased service needs, subject to available funding and review and approval by County Counsel.

Executive Summary:

The Department of Health Services (DHS or the Department) Public Health Disease Control Section plays a critical role in preventing and controlling the spread of infectious diseases in Sonoma County, particularly Hepatitis C (HCV), Human Immunodeficiency Virus (HIV), and sexually transmitted infections (STIs). Through long-standing partnerships with community-based organizations (CBOs), the Department delivers essential services to vulnerable populations at risk for or living with these diseases. These services include prevention, testing, care, and linkage to treatment, ensuring that underserved communities have access to necessary support.

The Department is requesting Board delegated authority to execute agreements for the provision of HCV and STI Prevention and Collaboration services, funded through California Department of Public Health (CDPH) grants. These grants will support public health activities focused on monitoring, prevention, testing, and ensuring retention in care for high-risk individuals. Services will be provided in collaboration with local CBOs that serve Sonoma County’s most vulnerable populations.

The STI grant allocates an annual amount of $352,112 ($704,224 total) for a term of July 1, 2024 through June 30, 2026 and the HCV grant allocates an annual amount of $200,199 ($800,796 total) for a term of July 1, 2024 through June 30, 2028. Both grants require the county to allocate 50% of this award to a local community-based organization based on a competitive award process.

 

Discussion:

The Department’s Public Health Disease Control Section is responsible for preventing and controlling the spread of infectious diseases within Sonoma County. The Disease Control Section has a long-standing history of collaborating with CBOs to deliver essential services to individuals at risk for, or living with, HCV, HIV, and other STIs. These partnerships ensure that vulnerable populations in Sonoma County have access to critical prevention, testing, care, and support.

Funding

Funding for the Public Health Division’s STI program management and collaborative efforts is provided through grants from the California Department of Public Health. In June 2024, the California Department of Public Health notified the Department of an additional grant award of $352,112 annually ($704,224 total) for a term of July 1, 2024, through June 30, 2026.

Funding for the HCV Prevention program was initially secured in 2019, when the Governor’s Office allocated State general funds to support preventive services for HCV and STIs within local health departments. On August 23, 2024, the California Department of Public Health notified the Department of an additional grant award of $200,199 annually ($800,796 total) for a term of July 1, 2024, through June 30, 2028.

As per the requirements of both grants, the Department is obligated to allocate fifty percent of this funding to a local community-based organization through a competitive award process. The remaining amount supports program activities conducted by the Departments Public Health Division.

Procurement Process

On September 24, 2024, the Department released a Request for Proposal (RFP) for STI and HCV Prevention and Collaboration services. The RFP was distributed to registered entities and interested parties to ensure broad participation. Two proposals were received, each applying for one of the two focus areas: HCV Prevention and STI Prevention. After a comprehensive evaluation, the evaluation panel recommended awarding contracts to the two proposers: Face To Face/Sonoma County AIDS Network and Santa Rosa Community Health Centers.

The Department is requesting Board delegated authority to execute an agreement with Santa Rosa Community Health Centers for the provision of Sexually Transmitted Diseases Collaborative Services. The contract term will be from February 1, 2025, through June 30, 2026, with a total not-to-exceed amount of $249,455.

The Department is also requesting Board delegated authority to execute an agreement with Face To Face/Sonoma County AIDS Network for the provision of HCV Prevention and Control services. The contract term will be from February 1, 2025, through June 30, 2028, with a total not-to-exceed amount of $366,972.

Narrative Explanation of Fiscal Impacts:

The Fiscal Year 2024-2025 Adopted Budget includes this fiscal year’s agreements portions as follows: $91,256 for the Santa Rosa Community Health Center and $66,669 for the Face To Face/Sonoma County AIDS Network.  Projected FY 25-26 includes $158,199 and $100,101 respectively and FY 26-27 includes $100,101. The remaining $100,101 for Hepatitis C Virus Prevention and Control Services agreement with Face/Sonoma County AIDS Network will be included in FY 27-28.
