Executive Summary:
During the June 17, 2022 Budget Hearings for fiscal year 2022-2023, the Board of Supervisors approved a community budget request from Health Action 2.0 for three years of funding at $500,000 per year from County General Fund, to create a community-based collaborative action network focused on addressing systemic inequities. This request was brought forward by the Department of Health Services (hereinafter, the “Department” or DHS) and the founding board of Health Action 2.0 (fiscally sponsored by Community Partners - a non profit specializing in intermediary and fiscal sponsorship work with new organizations). This item seeks to adjust the 2022-2023 adopted budget with additional revenue and expenditures and to authorize the Health Department director or designee to enter into an agreement with Community Partners to establish the Health Action 2.0, which is an equity-centered and community-driven effort to address the greatest inequities in Sonoma County.
Discussion:
On May 3, 2022 the Board adopted a resolution dissolving the Health Action Council as approved by the Health Action Council in February, to support the transition to Health Action 2.0 - a Collaborative Action Network and public/private partnership between the County of Sonoma and community leaders across the County; and to endorse and champion the work of Health Action 2.0. The new Health Action (currently called Health Action 2.0) is leading the development of an equity-centered and community-driven Agenda for Action to address the greatest inequities in Sonoma County, as indicated in the Portrait of Sonoma County Update 2021 <https://upstreaminvestments.org/impact-make-a-change/portrait-of-sonoma-county>. Health Action 2.0 will support grassroots and systems-change efforts to help increase and align the County’s efforts to address these inequities.
Agreement with Community Partners
Community Partners is a 501(c)3 nonprofit organization whose fiscal sponsorship program includes 185 projects across the state. They provide administrative and financial services for nonprofit projects to get up and running. Their staff provide guidance, training, and connections to help project leaders achieve greater impact. Health Action 2.0 selected Community Partners as their fiscal sponsor after working closely with them as contract managers for a large grant received during Health Action work over the past five years. Community Partners will set up and support the organization until the Health Action board seeks their own non-profit status - when staffing and infrastructure are in place. Health Action 2.0 staff under the fiscal sponsorship of Community Partners will do the following:
• Rebrand and launch Health Action 2.0 as an independent community-led organization that uses an interim board and fiscal sponsor in the short term.
• Develop the Agenda for Action with recommendations co-created with communities most impacted by inequities.
• Gain buy-in and commitment from policymakers, institutions, and decision-makers to endorse and share accountability in activating the Agenda for Action.
• Identify the first system-level intervention and develop the launch plan.
• Launch, support, and resource local action groups that define change efforts relevant to those local action groups.
Working with Existing Health Action Chapters and Local Grassroots Efforts
As part of Health Action 2.0’s foundational value of centering equity and community voice, local action groups are essential to the success of Health Action 2.0. There are many community-based groups in Sonoma County doing impactful work that are trusted in their communities, understand needs, and can address gaps, yet have not traditionally been included in institutional and government efforts to improve health, well-being, and equity. Health Action 2.0 seeks to be a leader in continuing to help bridge this gap between on-the-ground trusted groups and institutions.
Health Action chapters have been an integral part of Health Action for the past 10 years. There are approximately seven still-existing chapters -all unique in their identities, scope and purpose, yet all have provided a valuable hub for communication and services within their communities and helped to link the work of Health Action across the County. Health Action 2.0 is committed to continuing to lift the chapters’ work and support their evolution through this time of transition by continuing to provide convening support though the 2022-23 fiscal year. The existing Health Action chapters have been an important part of this transitional time for Health Action and are committed to exploring their own evolution, as needed, as well as to centering those populations and communities most impacted by the systemic inequities in Sonoma County and aligning with Health Action 2.0’s aspiration and values.
The Community Partners’ Health Action 2.0 budget includes funding to support and amplify collaborative local action groups at the grassroots level around key priorities identified in the Agenda for Action. Funding will be allocated through a focused approach to utilize local groups and chapters knowledge and expertise. This will include opportunities for existing chapters as well as grassroots efforts not previously part of Health Action and frequently excluded from collaborative efforts and will emphasize grassroots efforts working in areas and with populations experiencing the greatest disparities (as informed by the 2021 Portrait of Sonoma Update). A key difference in the work of Health Action 2.0 from Health Action 1.0 is centered in a targeted approach to specific issues that will be identified in the Agenda for Action process, rather than attempting to address a broad set of issues identified by leaders and experts. This shift means that funding and involvement from various chapters and local action/grassroots groups will not remain static over time but change as community-driven priorities evolve.

On June 17, 2022 the Board adopted the fiscal year 2022-2023 budget and authorized the County Administrator and Auditor-Controller-Treasurer-Tax Collector to make necessary budgetary adjustments, which included a three-year, $1,500,000 commitment to Health Action 2.0 via a Community Budget Request.
On May 3, 2022 the Board adopted a resolution dissolving the Health Action Council effective May 3, 2022; as supported by the Health Action Council to support the transition to Health Action 2.0 - a Collaborative Action Network and public/private partnership between the County of Sonoma and community leaders across the County; and to endorse and champion the work of Health Action 2.0.

Appropriations are being added with this item for Health Services and Non-Departmental Budget to process the transfer and record the expense. Future year’s transfers and expenditures will be included in the recommended budget.
