A)                     Authorize the Director of Health Services, or designee, to execute a grant agreement with Partnership HealthPlan of California for the Community Health Assessment and Community Health Improvement Plan project, to provide funding to support the Department of Health Services project management, community engagement, data analysis, and coordination with health system partners for a total not to exceed $100,000 through June 30, 2028. Approval includes acceptance of non-standard County contract terms, including sole indemnification of the Grantor by the County, binding arbitration provisions, and designation of Solano County as the venue for any legal claims, subject to review and approval by County Counsel.

B)                     Authorize the Director of Health Services, or designee, to execute modifications to the agreement in Recommended Action A in order to address changes in reimbursement methodologies, subject to review and approval by County Counsel.

Executive Summary:

This item requests Board approval to execute a grant agreement in the amount of $100,000 with Partnership HealthPlan of California (Partnership) to receive grant funding supporting the Sonoma County Department of Health Services’ (DHS) Community Health Assessment and Community Health Improvement Plan (CHA-CHIP) initiatives.

Under new California Department of Health Care Services (DHCS) requirements, Managed Care Plans (MCPs), which serve Medi-Cal members, contribute to local CHA-CHIP activities.  Partnership funding will support Department of Health Services project management, community engagement, data analysis, and coordination with health system partners.

 

Discussion:

In 2022, the California Department of Public Health established Future of Public Health (FoPH) funding to support Local Health Jurisdiction investments in local workforce and infrastructure. FoPH aims to transform and modernize six key service areas:

                     Workforce Development, Recruitment, and Training

                     Emergency Preparedness and Response

                     IT, Data Science, and Informatics

                     Communications, Public Education, Engagement, and Behavior Change

                     Community Partnerships

                     Community Health Improvement

As one of the requirements of receiving FoPH funding, Local Health Jurisdictions must submit a three-year Local Public Health Workplan and an annual Spend Plan. These plans must be informed by a Community Health Assessment and Community Health Improvement Plan (CHA-CHIP). As a result, the CHA and CHIP are now standard tools expected from all health jurisdictions. DHS completed its first CHA and CHIP in July 2025. 

Additionally, in an effort to align and streamline health planning and collaboration, the California Department of Health Care Services now requires all Medi-Cal Managed Care Plans to fulfill their Population Needs Assessment requirement by participating in the local, collaborative CHA-CHIP process in counties where they have contracts. California Department of Health Care Services defines participation to include contributing resources to the development of the CHA-CHIP. Sonoma County is served by two Medi-Cal Managed Care Plans: Partnership HealthPlan of California and Kaiser Foundation Health Plan, Inc. DHS has participated in joint monthly meetings with Partnership and Kaiser since May 2024 to engage in collaborative planning and implementation of shared goals and strategies associated with the 2024-2025 CHA-CHIP, as well as to strengthen this collaboration for future CHA-CHIP formulation cycles.

Partnership HealthPlan CHA-CHIP grant:

The Partnership CHA-CHIP grant addresses the ongoing need for strengthened Local Health Jurisdiction infrastructure and planning capacity. The CHA-CHIP development grants are designed to provide financial and technical support for the completion of the CHA-CHIP plan, in alignment with the California Department of Health Care Services requirement described above.

The grant, totaling $100,000, will offset staffing costs associated with overseeing the CHA-CHIP process. Specifically, the grant will be used to offset DHS’ staff costs related to project management, administration, data collection and analysis, facilitation, community engagement, data infrastructure and technology needs, communications, and technical assistance.

DHS will convene a countywide collaborative to engage diverse stakeholders across public health, healthcare systems, community organizations, and local government, in the development of ongoing CHA-CHIP work. This collaborative group will inform a comprehensive, inclusive CHA-CHIP process that identifies critical community health needs, establishes evidence-based priorities, and drives measurable, equity-focused outcomes for the Sonoma County community. Funds from these grants will support the administrative and management costs associated with these engagement efforts.

On December 16, 2025, the board authorized delegated authority to execute a Community Health Assessment and Community Health Improvement Plan grant agreement with Kaiser Foundation Health Plan, Inc. and with California Department of Public Health.

On June 13, 2025, the Board adopted the FY 2025-2026 Budget.

On February 6, 2024, the Board approved receipt of a progress report on Community Health Assessment and Community Health Improvement Plan.

Narrative Explanation of Fiscal Impacts:

A total of $100,000 in grant funding would be resolved to support the CHA-CHIP process in Sonoma County for Fiscal Year 2025-2026 through Fiscal Year 2027-2028:

                     Partnership HealthPlan of California Grant- $100,000 over three years (FY 2025-2026 through FY 2027-2028), The funding will support salary and benefit costs over this three-year period that is included in the FY 2025-2026 Adopted Budget and the FY 2026-2027 and future years’ Recommended Budget.
