Executive Summary:
Approval of this item will authorize receipt in fiscal year 2021-2022 of $3,117,102, representing $1,644,856 in Sonoma County Continuum of Care-designated State of California Homeless Housing, Prevention and Assistance 2 (HHAP-2) grant funds and $1,472,246 in County-designated State HHAP-2 funds. The California State Business, Consumer Services and Housing Agency (BCSH), through the Homeless Coordinating and Financing Council, provides this HHAP funding. HHAP is a $650 million block grant program designed to support regional coordination and expand or develop local capacity to address immediate homelessness challenges throughout the state. Spending must be informed by best practices focused on moving homeless individuals and families into permanent housing and ensuring those individuals and families maintain their permanent housing.
State HHAP-2 Continuum of Care funds were approved by the Continuum of Care Board in January 2021 to support core system of care programs in permanent supportive housing, rapid rehousing, and emergency shelter. These projects include support for the largest shelters in Sonoma County as well as permanent supportive housing projects serving chronically homeless single adults as well as transition age youth. Rapid rehousing projects serve single adults, families, and transition age youth across Sonoma County. These projects were also approved by the Board of Supervisors with additional support from local funds at the June 8, 2021 Board of Supervisors meeting. The approval of this Board Item ensures that the Community Development Commission has the authority for the receipt of the funds and to execute the agreements with the State.
Discussion:
Homeless Housing, Prevention and Assistance (HHAP) grant funding is authorized by AB 101 (Committee on Budget, Chapter 159, Statutes of 2019), which was signed into law by Governor Gavin Newsom on July 31, 2019. The County of Sonoma submitted separate Continuum of Care and County HHAP-2 applications for the respective allocations on November 5, 2020. Both applications were approved and awarded by BCSH in June 2021.
Two separate allocations are available; one to the Continuum of Care, with approval from the Continuum of Care Board (CA-504) and one available to the County of Sonoma. Separate applications were required to the State of California Business, Consumer Services and Housing Agency (BCSH). Funds under the purview of the Continuum of Care have been allocated to permanent supportive housing, rapid rehousing, and emergency shelter programs in the local system of care. Funds under the purview of the County of Sonoma will support outreach and engagement efforts of the Interdepartmental Multi-Disciplinary Team (IMDT), emergency shelter services and operations, and permanent housing services and operations at the two Project Homekey sites (Mickey Zane Place & Elderberry Commons).
On January 15, 2021 the Sonoma County Continuum of Care Board approved the application for $1,644,856 in funds that the State has designated for the Sonoma County Continuum of Care. This application included funds for local projects including rapid rehousing, emergency shelter, and permanent supportive housing. The Community Development Commission submitted the required request for these already-allocated funds, as well as the County allocation in late January. The State approved both application allocations and on June 8, 2021, the Sonoma County Board of Supervisors approved local contracts including HHAP-2 Continuum of Care funds.
The Sonoma County HHAP-2 application was submitted simultaneously with the Continuum of Care HHAP application, subsequent award notification from BCSH was received in late May, 2021. The HHAP-2 County grant includes support for street outreach, emergency shelter and permanent housing solutions. This grant will fund permanent supportive housing (PSH) operations and services at the Sonoma County Homekey PSH sites, located at the former Azura Hotel (known as Mickey Zane Place) providing 44 units of housing and the former Sebastopol Inn (known as Elderberry Commons) providing 31 units of housing. Funds will be utilized for operations and supportive services at these Homekey PSH sites. Both sites began operation in late 2020/early 2021.
The County also proposed HHAP-2 funding to support staffing and continuation of critical outreach services via the Homeless Encampment Access & Resource Team (HEART) and Interdepartmental Multi-Disciplinary Team (IMDT). The County HHAP-1 supports the early efforts of the County's ACCESS Sonoma initiative, the HHAP-2 is a continuation of that support. ACCESS Sonoma has a four-pronged approach; an IMDT staffed by representatives from all of the Safety Net Departments, an Integrated Data Hub/Watson Care Manager (WCM) developed in partnership with IBM, a system of governance led by the County’s Safety Net Collaborative, and partnerships with community-based organizations and academic institutions. The result is coordinated care from across our Safety Net Departments for our most vulnerable residents.
The Board is requested and recommended to authorize these agreements and acceptance of HHAP-2 funding, accepting $1,644,856 in Sonoma County Continuum of Care-designated State Homeless HHAP-2 grant funds and $1,472,246 in County-designated State HHAP-2 funds.
Prior Board Actions:
On June 8, 2021 (Item 83) the Board authorized funding for State HHAP-2 Continuum of Care Funds in local contracts.
On April 7, 2020 (Item 6) the Board authorized funding of $6,696,830.65, representing $3,476,293.48 in Home Sonoma County-designated State Homeless Housing, Prevention and Assistance (HHAP) grant funds and $3,220,537.17 in County-designated State HHAP.

Narrative Explanation of Fiscal Impacts:
Expenses: Appropriations for FY 2021-2022 are already included in the FY 2021-2022 budgets. Appropriations include funds from HHAP-2 (Continuum of Care) for local homeless programs in emergency shelter, permanent supportive housing, and rapid rehousing and funds from HHAP-2 (County) for IMDT outreach and non-congregate shelter support, and treatment beds.
