A)                     Authorize the Director of Health Services, or designee, to execute an agreement with Santa Rosa Community Health to provide sexually transmitted infections prevention and collaboration services for the period of three years with an option of two one-year extensions, for an annual contract amount of $135,160 and a total not-to-exceed amount of $675,800.

B)                     Authorize the Director of Health Services, or designee, to execute modifications to the agreements in this Board Item which do not significantly change the scopes of service in order to address increased service needs, payment or reimbursement methodologies, subject to available funding and approval by County Counsel.

Executive Summary:

The Department of Health Services seeks Board authorization to execute an agreement with Santa Rosa Community Health to provide sexually transmitted infections prevention and collaboration services for the period of three years with an option of two one-year extensions, for an annual contract amount of $135,160.

Following a competitive Request for Proposals process, DHS identified Santa Rosa Community Health as the proposer best qualified to provide community-based outreach, prevention, testing, care coordination, and linkage-to-treatment services for populations disproportionately impacted by sexually transmitted infections, Human Immunodeficiency Virus, and Hepatitis C Virus. The proposed agreement will support implementation of evidence-based prevention strategies funded through California Department of Public Health grants and advance County efforts to reduce communicable disease transmission, improve access to care, and promote health equity.

The Department of Health Services also requests authority for the Director of Health Services, or designee, to execute non-substantive amendments to the agreement, subject to County Counsel review and approval.

 

Discussion:

The Department of Health Services (DHS), Public Health Division Disease Control Unit, works to prevent and control communicable diseases through surveillance, investigation, prevention, and community engagement activities. Sexually transmitted infections (STIs), Human Immunodeficiency Virus (HIV), and Hepatitis C Virus (HCV) continue to present significant public health concerns and disproportionately impact populations experiencing barriers to healthcare access.

Many individuals at highest risk for infection face challenges accessing traditional healthcare services due to housing instability, transportation barriers, substance use disorders, lack of primary care access, and other social determinants of health. Community-based prevention, testing, navigation, and linkage-to-care services are critical strategies for reaching these populations, reducing disease transmission, and improving health outcomes.

To support these efforts, DHS released a Request for Proposals (RFP) on March 18, 2026, seeking a qualified community-based organization to provide STI prevention and collaboration services funded through California Department of Public Health grants. Required services included outreach and prevention education, testing and screening, linkage to care, treatment support, partner services, community collaboration, data collection and reporting, and activities designed to reduce barriers to care among populations at elevated risk for infection.

Proposals were due on April 30, 2026.  Two proposals were received and evaluated by a review committee consisting of representatives from Department of Health Services and Human Services Department. Evaluation criteria included organizational qualifications and experience, proposed service approach, ability to reach priority populations, program capacity, reporting capabilities, and cost effectiveness.

Following evaluation of the proposals, the review committee recommends award to Santa Rosa Community Health.

Under the proposed agreement, Santa Rosa Community Health will provide community-based prevention, testing, outreach, navigation, and care coordination services designed to increase access to STI-related services among populations at elevated risk of infection. Services will utilize low-barrier engagement strategies and will be coordinated with existing County and community-based prevention efforts to improve health outcomes and reduce disease transmission throughout Sonoma County. The contractor will also collaborate with County programs and community partners to support integrated approaches to communicable disease prevention and health equity.

DHS is requesting authorization to execute an agreement with Santa Rosa Community Health to provide sexually transmitted infections prevention and collaboration services for the period of three years with an option of two one-year extensions, for an annual contract amount of $135,160; and to execute amendments for non-substantive changes, subject to review and approval by Counsel.

Prior Board Actions:

On February 4, 2025, the Board authorized the Director of Health Services to execute an agreement with Santa Rosa Community Health Centers for a term of February 1, 2025, through June 30, 2026, to perform Sexually Transmitted Diseases Collaborative Services for a total not-to-exceed amount of $249,455 and to execute an agreement with Face To Face/Sonoma County AIDS Network for a term of February 1, 2025, through June 30, 2028, to perform Hepatitis C Virus Prevention and Control Services for a total not-to-exceed amount of $366,972.

On June 13, 2022, the Board authorized the Director of Health Services to execute agreements with Santa Rosa Community Health Centers for Hepatitis C Virus Prevention and Control ($163,424) and Sexually Transmitted Diseases Collaborative Services ($164,581) and Face2Face for Hepatitis C Virus Prevention and Control ($20,000) for services provided in fiscal years 2019-2020 and 2020-2021, for a total of $348,005.

Narrative Explanation of Fiscal Impacts:

The FY 2026-2027 Adopted Budget includes $135,160 in appropriations to fund the first year of the agreement with Santa Rosa Community Health.  The agreement has an initial three-year term, with the option to extend for up to two additional one-year extensions.  The annual contract amount is $135,160, for a total not-to-exceed amount of $675,800. 

 

Funding for this agreement is provided by the California Department of Public Health, Sexually Transmitted Diseases Control Branch, to support sexually transmitted infection prevention and control activities for the term beginning July 1, 2026, through June 30, 2031. Funding and expenditure appropriations for future years will be included in the appropriate annual budgets.
