Executive Summary:
The Human Services Department is requesting that the Board authorize the Director of the Human Services Department to execute a grant agreement from the California Department of Aging to fund the Aging and Disability Resource Connection, known in Sonoma County as the Aging and Disability Resource Hub, for the period of July 1, 2026, through June 30, 2027. Funding will support the core partners, Sonoma County Area Agency on Aging and Disability Services & Legal Center, to provide the administration and core services for the Aging and Disability Resource Hub.
Discussion:
The Human Services Department (HSD) is requesting authorization for the Director of Human Services to execute an agreement with the California Department of Aging (CDA) for $297,150 to fund the Aging and Disability Resource Connection, known in Sonoma County as the Aging and Disability Resource Hub (ADRH), for the period of July 1, 2026, through June 30, 2027. The ADRH is the local No Wrong Door system for older adults, people with disabilities and caregivers, designed to improve access to long-term services and supports through coordinated, person-centered information, assistance, referral, and service navigation.
The ADRH is led by its two core partners, the Sonoma County Area Agency on Aging (AAA), operated within HSD’s Adult and Aging Division, and Disability Services & Legal Center (DSLC), Sonoma County’s designated independent living center. The grant funds support administration and core ADRH services, which includes funding for a Program Planning & Evaluation Analyst, an Adult & Aging Supervisor, and an Adult & Aging Social worker. The funding also supports subcontracted direct services staffing and administrative functions including coordination across partner agencies, streamlined access to services, and warm hand-off referrals that help connect community members to the appropriate programs and supports.
Approval of this item will allow HSD to continue operation of the ADRH and maintain coordinated access to services for older adults, people with disabilities, and caregivers in Sonoma County. Without these funds, the associated positions and administration of the ADRH network could not be sustained.

Narrative Explanation of Fiscal Impacts:
HSD is not requesting to increase appropriations in Fiscal Year 2026-27. The revenue from the California Department of Aging was already included in HSD’s Fiscal Year 2026-27 Adopted Budget.
