Executive Summary:
The Department of Health Services provides legally-mandated medical care to juveniles in detention and children in protective custody at Juvenile Hall and the Valley of the Moon Children’s Center. Since July 2022, Medical Director services have been delivered by Santa Rosa Community Health Centers under a three-year agreement, followed by a short-term bridge contract (July–November 2025) to ensure continuity while DHS conducted a competitive procurement. A Request for Proposals issued in March 2025 resulted in one proposal from Santa Rosa Community Health Centers, which was evaluated and recommended for award. The Department of Health Services now seeks Board approval to execute a new five-year agreement with Santa Rosa Community Health Centers, effective December 1, 2025, through November 30, 2030, in an amount not to exceed $551,197.
Discussion:
The Department of Health Services (hereinafter “DHS” or “the Department”) is responsible for ensuring the health and well-being of all residents, including vulnerable populations such as juveniles in detention and children in protective care. In collaboration with the Probation Department and the Human Services Department, DHS provides and oversees the provision of medical services at Juvenile Hall and the Valley of the Moon Children’s Center (VOMCC). These services are mandated by law and are essential for maintaining the health standards required in such facilities.
The services are currently provided by DHS staff with Medical Director oversight and services being provided by Santa Rosa Community Health Centers. In July 2022, the Board approved a three-year agreement with Santa Rosa Community Health Centers for the Medical Director services. In June 2025, DHS executed a short-term bridge agreement (July 1 – November 30, 2025, $44,335) to maintain uninterrupted care while completing a competitive procurement.
On March 11, 2025, the Department released a Request for Proposals (RFP) for Medical Director services at the Juvenile Hall and Valley of the Moon Children’s Center with proposals due April 22, 2025. The RFP was distributed to all individuals and entities registered in the Sonoma County Portal and Purchasing Govdelivery Notification System in addition to a list of interested parties provided to Purchasing by DHS. Although the RFP was widely distributed, only one proposal from Santa Rosa Community Health Centers was received. The Evaluation Panel completed its review of the proposal and has recommended that the award be granted to Santa Rosa Community Health Centers.
The Department is requesting authorization for the Director of Health Services, or designee, to execute an agreement with Santa Rosa Community Health Centers to provide public health and medical director services to Juvenile Hall and Valley of the Moon Children’s Center for the period December 1, 2025 through November 30, 2030, in an amount not to exceed $551,197; and to authorize the execution of amendments for non-substantive changes subject to review and approval by counsel.

Prior Board Actions:
On June 3, 2025, the Board of Supervisors authorized the Director of Health Services, or designee, to execute an agreement with Santa Rosa Community Health Centers to provide public health and medical director services to Juvenile Hall and Valley of the Moon Children’s Center for the period July 1, 2025 through November 30, 2025, for an amount not to exceed $44,335; and authorized the Director of Health Services, or designee, to execute modifications to the agreement in this Board item which do not significantly change the scope of service in order to address increased service needs, subject to funding availability and review and approval by County Counsel.
On July 12, 2022, the Board of Supervisors authorized the Director of Health Services, or designee, to execute an agreement with Santa Rosa Community Health Centers to provide public health and medical director services to Juvenile Hall and Valley of the Moon Children’s Center for the period July 1, 2022 through June 30, 2025, in an amount not to exceed $307,092.

Narrative Explanation of Fiscal Impacts:
The FY 2025-2026 Adopted Budget includes sufficient appropriations to fund year one of this agreement with Santa Rosa Community Health Centers. This is a five-year agreement with a total not-to-exceed amount of $551,197. For Fiscal Year 2025-2026, the contract amount is $64,302.
This agreement is funded through Memorandums of Understanding with the Probation Department and the Human Services Department.
Future year funding and expenditures will be included in the appropriate year budgets.
