Executive Summary:
The Human Services Department is requesting that the Board adopt a resolution authorizing the Director to execute the Multipurpose Senior Services Program Standard Agreement with the State of California Department of Aging in the amount of $1,071,200 for fiscal year 2024/25 and authorize the Director to execute future contract amendments for subsequent contract years. The Multipurpose Senior Services Program provides home-based care management services to older adults at risk for nursing home or institutional placement, to reduce the risk of institutionalization.
Discussion:
The Human Services Department (HSD) receives funds from the State of California Department of Aging (CDA) to operate the Multipurpose Senior Services Program (MSSP) with the goal of preventing institutionalization and fostering independence and dignity for older adults.
MSSP provides case management services to Medi-Cal eligible older adults who are frail and at risk of placement in a skilled nursing facility due to significant functional support needs. MSSP serves approximately 200 individuals each month with care management services that are provided by a multidisciplinary team. The care management team coordinates medical and social services and arranges for additional long-term services and supports to be put in place. The Standard Agreement # MS-2425-11 with CDA for Fiscal Year (FY) 2024-2025 contains $1,071,200 in funding that helps pay for 4.0 full-time equivalent (FTE) Adult and Aging social workers, 1.5 FTE Senior Public Health Nurses, 1.0 FTE Social Work Assistant, and 1.0 FTE Adult and Aging Supervisor in MSSP.
