A) Authorize the Director of Health Services, or designee, to execute an agreement with Santa Rosa Community Health Centers to provide public health and medical services to Juvenile Hall and Valley of the Moon Children’s Center for the period July 1, 2025 through November 30, 2025, for an amount not-to-exceed $44,335.
B) Authorize the Director of Health Services, or designee, to execute modifications to the agreement in this Board Item which do not significantly change the scope of service in order to address increased service needs, subject to funding availability and review and approval by County Counsel.

Executive Summary:
The Department of Health Services (DHS or the Department) seeks Board approval to ensure continuity of critical medical and public health services for vulnerable youth populations in Sonoma County. The requested actions include authorizing the Director of Health Services, or designee, to execute a short-term bridge agreement with Santa Rosa Community Health Centers for services at Juvenile Hall and the Valley of the Moon Children’s Center (VOMCC) from July 1, 2025, through November 30, 2025, in an amount not to exceed $44,335. This bridge agreement will maintain uninterrupted care while DHS completes a competitive procurement process and finalizes a long-term agreement. Services will be fully funded through reimbursements from the Human Services Department and the Probation Department under an established memorandum of understanding.
The Probation Department is supportive of this bridge agreement.
Discussion:
Santa Rosa Community Health Centers
The County’s Department of Health Services is responsible for ensuring the health and well-being of all residents, including vulnerable populations such as juveniles in detention facilities and children in protective care. In collaboration with the Probation Department (Probation) and the Human Services Department (HSD), DHS oversees the provision of medical services at Juvenile Hall and the Valley of the Moon Children's Center (VOMCC). These services are mandated by law and are essential for maintaining the health standards required in such facilities.
On July 12, 2022, the Board authorized the Director of Health Services, or designee, to execute an agreement with Santa Rosa Community Health Centers to provide public health and medical services to Juvenile Hall and Valley of the Moon Children’s Center for the period July 1, 2022 through June 30, 2025, in an amount not to exceed $307,092. The agreement was based on a Request for Proposal process released in April of 2022.
The Department is currently in the process of securing proposals in response to a Request for Proposal posted to the Sonoma County Supplier Portal on March 11, 2025 with proposals due April 22, 2025. The RFP was distributed to all individuals and entities registered in the Sonoma County Portal and Purchasing Govdelivery Notification System in addition to a list of interested parties provided to Purchasing by DHS.
The Department expects the current procurement process to be completed in May of 2025and to return to your Board with a recommendation for a new, long-term service agreement in July of 2025. In the meantime, the Department is requesting approval of a bridge agreement to support uninterrupted services and to allow time to properly evaluate proposals and negotiate a final agreement.

Narrative Explanation of Fiscal Impacts:
Funding for the bridge agreement with Santa Rosa Community Health Centers will be fully covered through a memorandum of understanding between DHS and HSD and the Probation Department in the amount of $44,335. DHS maintains the agreement with Santa Rosa Community Health Centers and receives reimbursement from HSD and Probation for services rendered to each of the programs. These expenditures and revenues are included in the FY25-26 Recommended Budget.
