A)                     Authorize the County Executive or designee to execute a grant agreement with Community Action Partnership of Sonoma County on behalf of Health Action Together and NAACP Santa Rosa - Sonoma County Branch to provide $100,000 towards a community wellness hub in Santa Rosa.

B)                     Make a finding that the proposed expenditures are necessary to meet the social needs of the population of the County pursuant to Government Code Section 26227.

Executive Summary:

During the budget development process for fiscal year 2024-25, the Board approved Board Budget Request BOS-19, submitted jointly by Supervisors Hopkins and Gore, which consists of grant funding in the amount of $100,000 to the NAACP Santa Rosa - Sonoma County Branch and Health Action Together to support the Community Wellness Hub in Santa Rosa, a space for co-location of the two organizations and community service delivery.

 

This item authorizes the County Executive or designee to execute a new, revised grant agreement with Health Action Together and the NAACP (through fiscal sponsor Community Action Partnership of Sonoma County) and operationalizes the payment approved during budget, replacing the agreement approved by the Board on February 25, 2025 to change from a reimbursement award to a direct grant of funds. This action maintains the total funding provided by the Board to the Health Action Together since fiscal year 2022-23 to approximately $1.03 million.

 

The proposed grant agreement is authorized under Government Code Section 26227, pursuant to which the Board may fund projects deemed by the Board to be necessary to meet the social needs of the population of the County, including but not limited to, the areas of public health and welfare.

 

Discussion:

At the Fiscal Year 2024-2025 Budget Hearings, Supervisors Hopkins and Gore submitted Budget Board Request BOS-19 for consideration of funding. The request proposed funding for a Community Wellness Hub, for co-location of the NAACP Santa Rosa - Sonoma County Branch and Health Action Together organizations and a space to hold meetings, run programming, and offer services. On June 14, 2024, the Board approved Board Budget Request BOS-19, which consists of grant funding in the amount of $100,000 to NAACP and Health Action Together to support lease costs for a co-location for NAACP and Health Action Together at 1401 4st Street, Santa Rosa, and space to provide delivery of community services to the two organizations’ clients and hold community meetings and events.

 

On February 25, 2025, the Board approved a grant agreement for NAACP and Health Action Together, through their fiscal sponsor Community Partners.  In the intervening time, Community Action Partnership of Sonoma County (doing business as Sonoma Community Action Network) has replaced Community Partners as the fiscal agent for Health Action Together. Additionally, the terms of the agreement have been changed from reimbursement to a direct grant of funds to support operational costs of the Community Hub known as the HIVE.

The -NAACP Santa Rosa  Sonoma County Chapter <https://naacpsantarosasonomaco.org/> was established in 1953 and has a mission to ensure the political, educational, social, and economic equality of rights of all persons and to eliminate racial hatred and racial discrimination.

 

Health Action Together

Health Action Together <http://www.hatogether.org/> is a partnership of community leaders committed to ending health and social inequities in Sonoma County. People most impacted by racism are driving the organization’s efforts, including the development of an Agenda for Action created by the community. The Agenda for Action will identify the strategies needed to align partners and community resources to transform Sonoma County into a place where a person's skin color, zip code, or place of birth does not determine their health, education, or income.

 

In August 2007, the Board requested the Department of Health Services to convene a health action council (later called "Health Action") to work on improving health and health care for all Sonoma County residents. In 2022, the Health Action Council voted to transition to an independent nonprofit, now known as Health Action Together, under the fiscal sponsorship of Community Partners <https://communitypartners.org/>.

On February 25, 2025, the Sonoma County Board of Supervisors approved a grant agreement with Community Partners on behalf of Health Action Together and NAACP Santa Rosa - Sonoma County Branch to provide $100,000 towards a community wellness hub in Santa Rosa.

 

On June 14, 2024, at the conclusion of the Fiscal Year 2024-2025 Budget Hearings, the Sonoma County Board of Supervisors adopted a Concurrent Resolution adopting the Fiscal Year 2024-25 Budget and Position Allocation Listing and final Board direction for all entities governed by the Board of Supervisors/Directors/Commissioners.

Narrative Explanation of Fiscal Impacts:

The $100,000 grant to the NAACP Santa Rosa - Sonoma County Chapter and Health Action Together was approved during the Fiscal Year 24-25 budget hearings using one-time General Fund balance and will be rolled forward into FY 25-26. Appropriations will be programmed in the Q1 Consolidated Budget Adjustments.
