A) Adopt resolutions authorizing the Director of the Human Services Department to execute Standard Agreements totaling $3,065,659 with the State of California Department of Aging to increase available services for older adults, people with disabilities and their caregivers for supportive services, and authorize execution of future amendments to these Standard Agreements to adjust for revenue levels based on changes to funding:
i. Modernizing the Older Californians Act (Nutrition Only) Standard Agreement NM-2324-27 in the amount of $2,078,228 for the term of July 1, 2023, through March 31, 2029;
ii. Modernizing the Older Californians Act (Supportive Services Only) Standard Agreement OM-2223-27 in the amount of $869,516 for the term of January 1, 2023, through March 31, 2026; and
iii. Expanding Public Health Workforce Standard Agreement PH-2223-27 in the amount of $117,915 for the term of January 1, 2023, through September 30, 2024.
B) Adopt a position resolution effective September 1, 2023, to:
i. Convert 1.0 Full Time Equivalent time-limited Program Planning and Evaluation Analyst to permanent allocation in the Human Services Department; and
ii. Add 1.0 Full Time Equivalent time-limited Adult and Aging Social Worker in the Human Services Department to support the Linkages program through March 31, 2026.
C) Adopt a budget resolution to modify the Human Services Department Fiscal Year 2023-2024 Adopted budget to increase revenue and expenditure appropriations in the amount of $771,951. (4/5 Vote Required)

Executive Summary:
The Adult and Aging Division in the Human Services Department (HSD) provides safety net services to older adults, people with disabilities and their caregivers. The Older Californians Act Modernization funding is allocated to increase available services for older adults, people with disabilities and their caregivers for supportive services including Linkages case management and nutrition services.
The new revenue from the California Department of Aging (CDA) will allow the Adult and Aging Division to increase staffing by 1.0 Full-Time Equivalent (FTE) Adult and Aging Social Worker for the Linkages program, and convert 1.0 FTE time-limited Program Planning and Evaluation Analyst (PPEA) position to a permanent position. This increased staffing will address the unmet need for case management for adults with disabilities under the age of 60 and will allow for effective administration of the $3,065,659 through Fiscal Year (FY) 2028-29.
Discussion:
Funding Allocations
The Older Californians Act Modernization (OCAM) - Nutrition Only standard agreement (#NM-2324-27) totals $2,078,228 for the term of July 1, 2023 through March 31, 2029. This funding will be used for two purposes: (1) The allowable 10% administration funding will be used for the conversion of a 1.0 FTE time-limited PPEA to a 1.0 FTE permanent PPEA for a total of $207,823 (combined with the Older Californians Act - Supportive Services funding of $86,952 for a total of $294,775*); and (2) Funding will be made available through a competitive procurement process administered by HSD that will seek proposals from service providers for a variety of nutrition programs that may include intergenerational nutrition programs, restaurant meal programs, and other innovative programs to encourage older adults to improve their access to nutritional meals. The total available for this procurement is $1,870,405 through June 30, 2029.
The OCAM - Supportive Services standard agreement (#OM-2223-27) totals $869,516. This funding will be used for three purposes: (1) Funding a 1.0 FTE time-limited Adult and Aging Social Worker in the Linkages program from September 1, 2023 - March 31, 2026, using $372,908 (combined with $117,915 in Expanding Public Health Workforce (PHWF) funding for a total of $490,823); (2) Funding the conversion of a 1.0 FTE time-limited PPEA to a 1.0 FTE permanent PPEA using $86,952 (combined with the Older Californians Act - Nutrition Services funding of $207,823 for a total of $294,775*); and (3) Funding will be made available through a procurement process administered by HSD that will seek proposals from service providers for a variety of supportive services for older adults that include Foster Grandparent, Senior Companion, Aging in Place and others. The total available for this procurement is $409,656 through March 31, 2026.
The Expanding PHWF standard agreement (#PH-2223-27) totals $117,915 and is available through September 30, 2024. The total amount will be dedicated to funding a 1.0 FTE time-limited Adult and Aging Social Worker in the Linkages program (combined with the funding from the OCAM - Supportive Services allocation of $372,908 for a total of $490,823).
* The 1.0 FTE time-limited PPEA is currently funded through August 31, 2024, by CalFresh Healthy Living ongoing baseline funding (50%), and Mental Health Services Act (MHSA) - Innovations funding (50%) which expires August 31, 2024. HSD will use the new OCAM and PHWF funding of $294,775 starting September 1, 2024 to pay for the portion of this position that was previously funded through MHSA.
Positions
The 1.0 FTE PPEA position conversion will provide stability to the Adult and Aging Division in administering the $3,065,659 from the OCAM and PHWF funding through 2029. The position is also responsible for administering CalFresh Healthy Living funding and Dignity at Home Fall Prevention funding. Effectively leveraging these funding sources increases access for older adults to the array of services that promote better health through case management, fall prevention, and innovative nutrition programs.
The addition of the 1.0 FTE time-limited Adult and Aging Social Worker position in the Linkages program will allow for expansion of case management services to adults with disabilities age 18-59. Linkages historically has been funded by Older Americans Act Title IIIB that serves only adults 60 and older. There is a significant unmet need for case management services for the 18-59 age group making this an important case management program available in Sonoma County supporting this age group at no cost to clients. In calendar year 2022, Adult Protective Services (APS) received 582 reports of abuse and neglect for individuals aged 18-59. In the same time period, In-Home Supportive Services (IHSS) served 2248 individuals in this age group. While APS and IHSS provide valuable services to these individuals, their scope of services is limited and when these individuals need to be referred to case management, there have been limited to no options for referrals. The Linkages social worker will be able to serve up to 40 individuals at a time to provide approximately 6-months of support. The social worker will assess what individuals need to stay safe and healthy in the community and will work collaboratively with them to develop an individual care plan with goals to obtain the services they need.

OCAM - Nutrition Only funding in the amount of $2,078,228 was not budgeted for FY 2023-24. HSD requests approval to increase FY 2023-24 appropriations by $361,431 based on a term of 69 months as follows: FY 2023-24 $361,431 (12 months), FY 2024-25 $361,431 (12 months), FY 2025-26 $361,431 (12 months), FY 2026-27 $361,431 (12 months), FY 2027-28 $361,431 (12 months), and FY 2028-29 $271,073 (9 months).
OCAM - Supportive Services Only funding in the amount of $869,516 was not budgeted for FY 2023-24. HSD requests approval to increase FY 2023-24 appropriations by $316,188 based on a term of 33 months as follows: FY 2023-24 $316,188 (12 months), FY 2024-25 $316,187 (12 months), and FY 2025-26 $237,141 (9 months).
PHWF funding in the amount of $117,915 was not budgeted for FY 2023-24. HSD requests approval to increase FY 2023-24 appropriations by $94,332 based on a term of 15 months as follows: FY 2023-24 $94,332 (12 months) and FY 2024-25 $23,583 (3 months).

Narrative Explanation of Staffing Impacts (If Required):
Adult and Aging Social Worker: Based on the Older Americans Act criteria, the current Linkages model services those 60 and older. The new funding from the Older Californians Act will allow the Linkages program to expand case management services to individuals with disabilities 18-59 years old. The addition of an Adult and Aging Social Worker position will increase the number of clients that can receive case management services.
PPEA: Converting the position to permanent will ensure the Adult and Aging Division will be able to lead and support community innovations and initiatives throughout the county. This position has resulted in increased funding awarded to the county to support healthy living initiatives and engagement with vulnerable older adults. A permanent position will strengthen nutrition and healthy living program initiatives due to the ability to develop deeper relationships and depth of knowledge required to braid Older Americans Act, OCAM, and CalFresh Healthy Living program funding that support pilot programs, contracts, and budget recommendations. Conversion supports program deliverables year-to-year for related and synergistic initiatives.
