Executive Summary:
On October 14, 2025, the Board of Supervisors approved a resolution authorizing the Department of Health Services to apply for up to $68 million in funding to construct a Peer Respite Facility, Psychiatric Health Facility, and Mental Health Rehabilitation Center. This amount included estimated construction costs and a contingency allowance.
During the week of October 20, 2025, DHS received updated cost information from the Public Infrastructure Department indicating that the total project cost is likely to be approximately $99.8 million. The Department of Health Services is therefore requesting Board approval of a revised resolution authorizing an increased funding application amount of up to $100 million.
Discussion:
On October 14, 2025, the Board of Supervisors approved a resolution authorizing the Department of Health Services (DHS) to apply for up to $68 million in funding to expand the County’s behavioral health continuum of care through the addition of a Peer Respite Facility, Psychiatric Health Facility, Mental Health Rehabilitation Center, or a combination of these facilities.
Since that approval, DHS has been working with Sonoma Public Infrastructure (SPI) to develop a detailed project budget for the grant application. Initial estimates indicated that construction of these specialty behavioral health beds would cost approximately $1.3 million per bed. Based on that figure, DHS built contingencies into the October 14 Board item and requested authorization to apply for up to $68 million from the California Department of Health Care Services (DHCS).
As previously reported, DHS continues to collaborate with County partners to identify a project site, refine cost estimates, and address match and other key application criteria ahead of the October 28, 2025 application deadline.
On October 21, SPI provided updated data showing that comparable projects in the Bay Area and Sacramento regions range from $1.2 million to $1.5 million per bed. These fluctuations, combined with inflation and rising construction costs, have increased the anticipated total project cost. Accordingly, DHS is revising its application to reflect the most current cost projections of $99.8 million.
DHS now seeks Board approval to amend the previously authorized funding amount and increase the maximum request to DHCS under the Behavioral Health Continuum Infrastructure Program (BHCIP). This revision will ensure the County’s application accurately reflects updated cost estimates and minimizes the risk of cost overruns. The updated Board Authorizing Resolution will be provided to DHCS post-application submission, given that the applications were due, and submitted by DHS, on October 28, 2025.

Narrative Explanation of Fiscal Impacts:
Approximately $100,784 will be incurred as part of the application process. This includes $78,643 to Sonoma Public Infrastructure for assistance with real estate, construction, and schematic portions of the application and $22,141 to the Glenn Price Group for grant writing services. These expenses, which are included in the Fy25-26 Adopted Budgeted, will be covered using 2011 Realignment funds and will be included as sunk costs towards the grant match.
