A)                     Authorize the Purchasing Agent to execute two Blanket Purchase Order Agreements for as-needed medical waste management services Agreements to be for one (1) year with options to extend for up to four (4) additional one (1) year periods.

B)                     Authorize the Purchasing Agent, and designees, to issue work requests and option extensions pursuant to the agreements, contingent on availability of sufficient funding for such services.

Executive Summary:

General Services is proposing the County enter into multiple Blanket Purchase Order Agreements to make medical waste management services available on an as-needed bases for all County departments and affiliated entities. The Agreements have no guaranteed minimum amounts of usage and will be used on identified need. The Agreements will facilitate ease of access and achieve administrative efficiency for departments using medical waste management services.

 

Discussion:

In the course of delivering services to the public, County departments generate medical waste and therefore require specialized medical waste management services. Members of the public and county employees requiring certain self-administered medical treatments may also generate medical waste at County facilities. Medical wastes include materials classified as being potentially biohazardous (“red bag waste”), sharps, and pathology wastes, as well as pharmaceutical wastes, including controlled substances. All Medical Waste must be managed in strict compliance with Federal, State, and local regulations including, but not limited to:

 

• California Health and Safety Code Sections 117600 to 118360, the California Medical Waste Management Act

• Title 49 of the Code of Federal Regulations, Sections 100-183 (US Department of Transportation labeling and transportation requirements)

• Title 8 of the California Regulations Sections 5193 Blood-borne Pathogens and 5192 Hazardous Waste Operations and Emergency Response (as they apply to the medical waste management industry).

 

Countywide emergencies and disasters have resulted in sudden increases in medical waste volumes and/or changes in locations of medical waste collection and staging for supplier pickup. The proposed Agreement will establish a pre-positioned contract with a qualified supplier of medical waste management services.  The County previously had a contract with Asepsis Bio Group for such services.

 

Current medical waste needs are as follows:

                     Monthly service at the Valley of the Moon Children’s Center

                     Monthly service at Animal Services

                     Monthly service at multiple Behavioral Health locations

                     Bi-weekly service at the Public Health Lab

                     Weekly service at the Coroner’s Office

                     As needed service at the Juvenile Hall

                     As needed service at other Health Services locations

                     As needed service at disaster related shelters/locations

 

Procurement

The Purchasing Division issued a Request for Proposal (RFP) soliciting submittals from three hundred and ninety-two (392) potential bidders for medical waste management services. Three (3) proposals were received and evaluated based on the following criteria: ability to perform services; professional qualifications; demonstrated history of providing similar services to comparable entities; quality of work as verified by references; and cost relative to scope of services.

 

The General Services Purchasing Agent recommends that the County enter into two Blanket Purchase Order Agreement with Clean Harbors based on the evaluation committee recommendations.  One agreement will be for non-disaster usage.  The other agreement will be for use during a disaster.

Narrative Explanation of Fiscal Impacts:

Exact fiscal impact depends on the actual use of these services.  The estimated cost of these services is under $30,000 annually.  Prior medical waste services total expenses in the past three (3) years have been as follows: FY 19/20 $22,275.94, FY 18/19 $20,309.34, and FY 17/18 $13,137.96.
