Authorize the Director of Health Services, or designee, to execute Board direct designated Fiscal Year 2024-2025 funding allocations to provide services necessary to meet the social needs of our community (Government Code - GOV § 26227) for a term of August 1, 2024 through June 30, 2025 including authority to execute modifications thereto which do not significantly change the scopes of service and/or the total funding, subject to review and approval by County Counsel, for the following agency partners:

A)                     California Parenting Institute dba Child Parent Institute to provide family resource services for a not-to-exceed amount of $247,810.

B)                     Northern California Center for Wellbeing to provide vaccine equity outreach services for a not-to-exceed amount of $300,000.

Executive Summary:

​​On June 14, 2024, consistent with California Code, Government Code - GOV § 26227 (CG 26227) during the budget development process for Fiscal Year 2024-2025, the following Board Budget Requests were approved as direct designation funding:

A)                     BOS-10 to direct a one-time support of $248,000 from Board of Supervisors available discretionary funds to the California Parenting Institute dba Child Parent Institute (CPI) to provide family resource services.

B)                     BOS-18, submitted by Supervisors Gore and Hopkins, which consists of a payment of $300,000 to the Northern California Center for Wellbeing to provide vaccine outreach services.

CG 26227 <https://leginfo.legislature.ca.gov/faces/codes_displaySection.xhtml?lawCode=GOV&sectionNum=26227.> allows for the Board of Supervisors to appropriate and expend money from the general fund or to fund programs deemed to be necessary to meet the social needs of the population.

​In order to disburse the Board-designated public funds, staff is requesting delegated authority for the Director of Health Services, or designee, to execute partner funding agreements for a term of August 1, 2024 through June 30, 2025. Delegated ​authority includes ability to execute modifications thereto which do not significantly change the scopes of service in either agreement and/or the total not-to-exceed amount, subject to review and approval by County Counsel. ​

Discussion:

​​Child Parent Institute (CPI) Reimbursement-Based Agreement

CPI is a parent education and children’s mental health agency serving families throughout Sonoma County since 1978. CPI provides child therapy, family resource assistance, parent education, facilitated supervised visitation and a non-public school providing adolescent special education/mental health services. 

​​At the Fiscal Year 2024-2025 Budget Hearings, Supervisor Coursey submitted Budget Board Request BOS-10 for consideration of funding.  The request proposed funding for CPI’s Family Resource Center (FRC) at John Reed Elementary School (Attachment 2). On June 14, 2024, consistent with CG 26227 during the budget development process for fiscal year 2024-2025, the Board approved Board Budget Request BOS-10 which consists of a payment of $248,000 to CPI to provide family resource services.

CG 26227 allows for the county to appropriate and expend money from the general fund or to fund programs deemed by the Board of Supervisors to be necessary to meet the social needs of the population.

​The Department of Health Services is requesting the Board to delegate authority to the Director of Health Services, or designee, to execute an agreement with CPI to provide family resource services for a term of August 1, 2024 through June 30, 2025 and a not-to-exceed amount of $247,810.  Though BOS-10 authorized payment of $248,000 for this agreement, breakdown of total costs associated with the final agreement comes to $247,810. Through the proposed agreement, CPI will operate the FRC embedded within John Reed Elementary School, with Community Health Workers on site 5 days per week. The following services will be provided at the FRC over the course of the agreement:

                     ​Parent Education Classes and Individual Parent Education to strengthen protective factors. Health education through partnership with local service providers on site. 

                     ​Resource Navigation to increase parents’ knowledge and the ability to access existing community support systems where they can seek support in areas such as transportation, housing, economic assistance, employment services, health and nutrition, childcare, education, shelters, resources, medical care, and activities for their children.

                     ​Weekly Cafecitos to share information and develop strong community relationships. Cafecitos are informal coffee gatherings that can be a powerful way to build and strengthen the Latinx community. These gatherings can foster connection, provide support, and create a space for cultural exchange and shared experiences. 

Center for Wellbeing Lump Sum Disbursement Agreement

The Center for Well-Being has provided health and wellness services to Sonoma County since 1994. The Center for Well-Being’s mission is to improve the health and well-being of our community through evidence-based health practices and innovative programs that advance health equity for all, and their vision is to achieve improved community health through inclusive and just policies, programs, and services. For 30 years, the Center has conducted culturally responsive and linguistically appropriate public awareness and community outreach campaigns to reach our most vulnerable populations via trusted messengers. Their campaigns have included emergency preparedness education in response to Sonoma County floods and wildfires and vaccine hesitancy outreach, among other initiatives. During the pandemic, they participated in a regional cross-county outreach campaign to reach communities disproportionately impacted by COVID-19. The Center for Well-Being engaged residents in conversations to address vaccine hesitancy, provided education and resources for effective mitigation strategies, and linked individuals and families to testing and vaccination sites.

In June of 2024, the Center for Wellbeing requested $300,000 in one-time funds to support a vaccine outreach campaign. The goal of the campaign is to increase the resiliency of historically underserved communities by increasing connectedness to community resources and by reducing health disparities to protect the most vulnerable individuals in the community from COVID-19, flu, RSV, and other communicable diseases.

For this agreement Center for Well-Being leadership has requested a lump sum disbursement in order to secure and train Community Health Workers/Promotores de Salud staff, conduct outreach activities, and purchase vaccines. The Center for Well-Being will be obligated to provide quarterly supporting expense documentation with final documentation due by June 30, 2025.

On June 14, 2024, at the conclusion of the Fiscal Year 2024-2025 Budget Hearings, the Sonoma County Board of Supervisors adopted a Concurrent Resolution adopting the Fiscal Year 2024-25 Budget and Position Allocation Listing and final Board direction for all entities governed by the Board of Supervisors/Directors/Commissioners.
