A) Authorize the County Executive Officer, or designee, delegated authority to execute agreements and/or amendments, subject to review and approval as to form by County Counsel, as follows:
i) An interagency agreement with Sonoma County Special Education Local Plan Area, to coordinate medical therapy services between the Department of Health Services and the Sonoma County Special Education Local Plan Area effective upon execution and to automatically extend each year thereafter unless notified in writing by either agency.
ii) An amendment to The Regents of the University of California agreement, to provide professional medical services, extending the term of the agreement through July 30, 2028.
iii) Modifications to these agreements which may increase the total not-to-exceed amount to address increased service needs and/or will not significantly change the scope of services, subject to available funding and review and approval by County Counsel.

The Department of Health Services is requesting authorization to execute an interagency agreement to strengthen Health Care Coordination Services for medically vulnerable populations, including continued collaboration with the Sonoma County Special Education Local Plan Area to ensure coordinated therapy services for children with disabilities. Additionally, the Department seeks to amend an agreement with the Regents of the University of California to extend physician-provided medical therapy services to Sonoma-based clients through July 30, 2028, at no additional cost. Together, these agreements support efficient, equitable, and legally compliant service delivery to both animals and medically vulnerable residents of Sonoma County.

Health Care Coordination Services (CCS) Medical Therapy Unit - Sonoma County Special Education Local Plan Area - Interagency Agreement (IAA)
The Sonoma County Health Care Coordination Services (CCS) Medical Therapy Unit connects Sonoma County’s medically vulnerable populations to essential healthcare services, ensuring continuity of care for those with complex health needs. The program aligns with Medi-Cal managed care requirements, public health case management standards, and local health equity initiatives.
CCS and the Sonoma County Special Education Local Plan Area (SELPA) have a long-standing collaborative relationship to provide therapy services to children with medically and educationally necessary needs. A prior Interagency Agreement (IAA) between the County and SELPA was signed in 2007 and extended in 2009 on an auto renewal.
The State of California has recently approved a new local template for such agreements. This IAA updates and formalizes the relationship based on the new state template, ensuring compliance with current requirements and providing clarity for both parties to facilitate efficient and coordinated provision of therapy services.
The IAA supports continuity of care, avoids duplication of services, and helps ensure that children with disabilities receive both the educationally necessary and medically necessary therapy services to which they are entitled under the law. The IAA also clarifies the Sonoma County Special Education Local Plan Area’s (SELPA) responsibility for sites and supplies to the CCS Medical Therapy Program. There is no cost from the County to SELPA.
The Agreement shall be effective from effective date and shall automatically extend annually unless either party provides written notice of termination. Modifications require mutual written consent.
Health Care Coordination Services (CCS) Medical Therapy Unit - The Regents of the University of California (UCSF) - Amendment
The Health Care Coordination program connects Sonoma County’s medically vulnerable populations to essential healthcare services, ensuring continuity of care for those with complex health needs. The program aligns with Medi-Cal managed care requirements, public health case management standards, and local health equity initiatives.
Under this Service Agreement, UCSF Physicians provide oversight of the CCS Medical Therapy Conferences, and the physical and occupational therapy provided on site, per State regulations and guidelines. UCSF retains authority and responsibility for providing UCSF Physicians to the County, including recruiting, hiring, promoting, compensating, and disciplining, as well as establishing the terms of the UCSF Physicians’ employment with UCSF. The partnership with UCSF alleviates health care barriers for the Sonoma County children with medically complex therapy needs, and provides necessary specialty care medical oversight.
The Department is seeking delegated authority to execute an amendment to the Agreement with The Regents of the University of California (UCSF) for the provision of Health Care Coordination Services within the Medical Therapy Unit. This amendment extends the term of the existing agreement, originally effective July 31, 2022, through July 30, 2025, for an additional five years through July 30, 2028. This amendment is at no cost to the County.
